This is Mechanism 131 from 507 Mechanical Movements. Assemble as shown in the video. Lay out the parts on the print bed as shown in the picture. You’ll need two C-clips (parts 2 and 6), which may require supports in the groove areas. I didn’t use supports, and it worked fine.
