Wanhao i3 Plus Bearing Spool Holder with increased clearance

This is a remix of jpotegal's spool holder.

Main differences are:
1) The spool holder has been raised around 25mm to provide more clearance for large spools. I needed this to avoid contact with my enclosure.
2) It's been narrowed to fit spools with diameters as low as 32mm, such as those that come with Cocoon Create printers.
3) The clearances on each side between bearing and spool holder have been shrunk to 0.5mm.
