Fanmigo for e3d hotend

This is a remix https://www.thingiverse.com/thing:2400384 adapted for use with an e3d hotend

improved:
- Logo on the front
- prints without supports and without the need of already having a layer fan
- adjustable (4mm) height

print with low infill (10% more than enough), front should be on the build plate when printing (orientation just like it is in the STL file)
