40mm fan mount E3D v6

First stage of integrated hotend and part cooling fan mounts that can be used with [Trinus E3Dv6 (Bowden) Precision Piezo](https://www.thingiverse.com/thing:2690892 "Trinus E3Dv6 (Bowden) Precision Piezo").

Next stage will be to create the part cooling fan mount and then add a mounting point on the hotend fan to hang the part cooling from.

The part will be designed to use 40mm fans on both mounts.
