The other hot end holders I have used take care to avoid touching the lower fins. If the nozzle drags at all it has plenty of leverage to move around.
This design grips the hot end as tightly as possible. From the thermal images I've seen of an e3d the heat at the lower fins isn't an issue for PLA. In practice this hasn't been an issue either. The trade off here is that if a print drags hard enough it can bend the heat break.
