Universal phone to microscope adapter remix

This is a remix of ggoss's camera to microscope adapter. The code has been modified to be easier to customize, and the adapter body and lower clamp have been joined into one piece. The original was split to make it possible to print on smaller printers, but most build volumes are more than large enough. It was also made long enough to accommodate larger phones, like the Google Pixel or Samsung Galaxy S6. The original was slightly too short for these.
