E3d Carriage horizontal parts rework

This is a rework of https://www.thingiverse.com/thing:2701617 a Horizontal quick exchange hot end.
- Fan to duct has been raised slightly since it sat a little low and didn't really give any good seal or fit.
- Mounting bracket also felt too flimsy. Not it actually cups around the carriage and is pretty rock solid.
