Extruder Cover and Knob for CR-10 metal top-end

This is a remix from graham01's original design modified for a metal top-end from Banggood.
I've added a clip for the EZABL from TH3D and rub block on the inside to keep the cover level when you press on it.
Simply add a small piece of PTFE tubing potted in place when completed.
Also included an Extruder thumb knob designed to the diameter in the cover.
