The covers for the bolts on my Winnebago RV seat belts kept falling off because they lacked anything to hold them in place. One of them eventually got lost, so I created replacements that don't fall off.
In image #1 the cover on the left is the retentionless factory OEM version. The one on the right is my version with tapered 1.0mm retention flanges added.
The #2 image shows the bolt head with the cover missing. Image #3 shows my cover held securely in place.
