Two Trees Sapphire S - hotend mount

Here's a remix of the great Rics's model, compatible with the original ducts.

Changes:
- 15mm depth radiator fan slot for 40x40x15 and 40x40x20 fans;
- two bars to close gaps between the mount and the ducts;
- new upper spacers to use stock bolts;
- radiator fan wires channel;
- more secure mount pads;
- new "precise" ducts.

Use DIN 125 washers to screw the mount to the radiator.

There's also a model with bigger fan hole, made per request, not tested.
