The original base was too short for my needs and the screw mount points interfered with the placement near the corner so I created a new, taller base for it.
The original triple-bearing assembly slips right into this new mount which also uses a standard 608zz bearing.
Works very well, easy to print, uses standard 608zz or 608rs bearings.
9/2/2019 Update: Added a 45 degree angle base version which works better for Ender 5 and my top-mounted extruder setup.
