Direct drive extruder for E3D-V6

Chopped a bit out of the original part to allow easier fitting of and E3D-V6. You can now pull the PTFE tubing up to the gear.

As a bonus I also made a hot-end mount that which allows adding 1 or 2 40mm regular fans if you don't have any centrifugal one available.
