This is a mount for an E3D v6 extruder. There is one file with and one without a mount for a 18mm auto level sensor.
To attach the counter part to the main part you need two M3 nuts and two M3x15mm bolts.
To mount the fan an the duct on the counter part you need two more M3 nuts and two M3x15mm bolts. Washers are not necessary, but recommended.
