This is a mount for attaching a dial gauge to the spindle on a desktop CNC router. This fits a spindle with a collet thread of 14mm. The gauge is a widely available type with a diameter of approximately 2 inches.
The gauge is mounted by removing the four screws that hold the back plate, removing the plate, and replacing it with this print. To connect the mount to the spindle, move the spindle away from the bed, remove the collet, slip on the mount and screw the collet back on to secure the mount.
To test the level of your bed (or more technically tramming) move the probe to a point over bed, lower the Z axis 1 to 2 mm and note the reading on the gauge. Move the probe to each corner and the center of the bed and note the readings. The differences between the readings indicate how far off your bed is from perpendicular to the spindle.
