I've designed this Dual-USB to RJ45 adapter for connecting two USB ports with my 3D-Printer (1x printer + 1x webcam) using only one patchcable (RJ45 Keystone-Module on the printer).
For this cable you need two USB-A plugs like these:

...and a normal RJ45 patchcable.
You have to trim the plugs like you can see on the picture (black markings). I've used a multitool with a small cutting blade.
Finally, glue the coverplates on it.
