Nintendo Famicom Disk System transport insert

I just sketched of a scan of the original cardboard insert. It has the same thickness as the cardboard one.

You might wanna insert it with the smoother side facing downward because the reading head is on the bottom.

As there isn't any complexity about the thing there aren't really any printing settings to recommend. Just use settings you used succesfully on flat things.
