I needed a mount for my volcano and 5015 fan, so I come with this design. It's not perfect, but it works. Have fun!
BOM:
M4x25 with conical head x2 - connect two parts together
M4x5 flat head x2 - for back mount (two from original mount)
M3x12 flat head x2 - for front mount
M3x12 flat head x2 - for bltouch (optional)
M4x20 flat head x1 - for 5015 fan
