Simple MK8 Extruder Lever

Using some spare parts to put another extruder together hit a snag when the lever arm was both too large for the space and had a thumb tab that prevents it from being mounted. The size/shape of the lever from https://www.thingiverse.com/thing:62510 was perfect, and the compact thumb tab from https://www.thingiverse.com/thing:650957 solved the last problem.
